This role provides direct support to families navigating child custody, child support, and related family court processes.
The Case Manager manages an active caseload of approximately 40 participants annually, helping them understand court requirements, connect to legal consultations and workshops, coordinate services, track progress toward reunification goals, and provide practical, relationship-based support throughout the process. This position is ideal for someone who believes in second chances, whole-family healing, and the importance of fathers and caregivers being positively involved in their children’s lives.
Key Responsibilities
- Conduct intake, assessment, and ongoing case management for parents and caregivers enrolled in the CCSNA program, managing a caseload of approximately 40 participants annually.
- Develop individualized service plans and update them based on participant goals and progress.
- Help participants navigate child custody, child support, and related family court systems.
- Support participants in understanding court requirements, paperwork, timelines, and next steps.
- Coordinate referrals to legal consultations, workshops, counseling, and other support services.
- Support coordination of weekly legal workshops and one-on-one attorney consultations as part of the CCSNA program model.
- Maintain regular contact with participants and provide encouragement, accountability, and follow-up.
- Document services, participant progress, and outcomes in required data systems and case files.
- Collaborate with attorneys, program staff, probation partners, and community-based organizations to support family reunification goals.
- Assist with outreach, workshops, and community engagement efforts across Alameda County.
- Provide warm handoffs to additional services including family support, counseling, and other stabilizing resources.
- Uphold confidentiality and handle sensitive family information with care and professionalism.
Qualifications
- Bachelor’s degree in social work, human services, psychology, legal studies, or a related field preferred.
- At least 2 years of experience in case management, family support services, legal navigation, reentry services, or a related field.
- Experience working with fathers, families, justice-impacted individuals, or communities impacted by court involvement strongly preferred.
- Strong understanding of trauma-informed, culturally responsive, and strengths-based practice.
- Ability to build trust with participants from diverse backgrounds.
- Strong organizational skills and ability to manage multiple cases, appointments, and follow-up tasks.
- Comfortable working collaboratively with attorneys, social service providers, and system partners.
- Clear written and verbal communication skills.
- Spanish-speaking or other bilingual ability is strongly preferred. Lived experience with reentry or system navigation is strongly valued.
What We’re Looking For
We are looking for someone who:
- believes families deserve support, not judgment
- can balance empathy with strong follow-through
- understands how systems can overwhelm families and wants to help make them more navigable
- values father involvement, co-parenting support, and family stability
- can work both independently and as part of a collaborative team
